In order to keep accurate records and to support you to the best of our ability there are several forms with client information needed. It is vital that this information stay current, should you have any changes in your health or contact information inform us immediately by filling out a new form with your updated information. Occasionally we may request a written doctors note in order to provide services. Please know that we have your health and wellness in mind and are striving to provide you with the highest quality care.

Pregnant clients must fill out both the prenatal massage information form and the informed consent form and submit it to Moon Cycle Arts 24 hours prior to a scheduled appointment for review. This is so that, should you have any contraindications that would prevent you from receiving the service we can discuss this before your appointment date. We want to ensure that your health and the health and wellness of your child is fully supported and always our first priority. This will also allow us time to communicate with your doctors office should we need to. Be aware that you will have to contact your doctor and possibly sign a waiver in order for them to discuss any conditions you may have with us.
