In order to reserve Placenta Encapsulation Services for your birth you will need to turn in the contract and client intake form.  There is also a waiver for the hospital that you can include with your birth plan, should the hospital need one.  When you have received your capsules after your birth, there is a survey I would appreciate you filling out.  Thank you for your time, I look forward to working with you.
